Your Care Plan

When Surrey Children's Service get involved in looking after you they have to write down what they will do to help you. This is called your Care Plan. It will say what Surrey Children's Service will do to help you with:

  • your health
  • education
  • religion
  • your culture
  • contact with family and friends
  • hobbies
  • interests
  • and any other needs you have.

For many young people your care plan will also say how your parents or guardians will help look after you.

A Placement Agreement will also be written and will say what happens day to day where you live. It will be about things like:

  • your living arrangements
  • getting to school
  • and your health.

It will also say:

  • who you will be able to see, or not see
  • how much pocket money you’ll get
  • what time you have to be in
  • your travel arrangements.

If you agree with your plan you will be asked to sign a form. You don't have to agree with the plan. Your parents or guardians should be involved in writing your care plan and placement agreement. You will be involved too if you are old enough and can understand what is happening. Your social worker and the people who will be looking after you should also help with the plans. Your social worker may need to talk to other people like your doctor or teacher about the best way to look after you. Your care plan and placement agreement should be written within 10 days of Children's Services helping to look after you.

What if I need help to take part?

Please talk to your social worker about this.

  • If your first language is not English your social worker can arrange for an interpreter to come to your review if you want.
  • If you have any speech or hearing difficulties your social worker will make sure you get the support you need.
  • You are welcome to bring someone with you such as a friend or advocate (someone who speaks up with you) to support you.

Once a care plan and placement agreement are written, you, your parents, guardians, and the people looking after you will be given copies.
